Other than the 1st player port, I've noticed the box has a hard time
recognising controllers. Why is this? I have an extremely old Xbox, but
it seems every Xbox has this issue, because several of my friends have
newer Xbox consoles and the same problems. I modify Xbox consoles as a
hobby, so I think I could fix the problem if I knew what was causing it.

Believe it or not, Xbox controller ports are just USB 1.1 sockets with a
non-standard connector. The earliest Xbox had the root USB hub as a
separate daughterboard, which was highly susceptible to static electricity
and anything that might cause a voltage spike. Later versions had the USB
controller consolidated onto the motherboard, but as a result have the
wiring running further. Also, the internal connectors have a tendency to
come loose for no apparent reason.

It's very easy to open up the Xbox, check the wiring and make sure that
you have good continuity. Note that Xbox controller ports have five wires
rather than four, although that fifth wire wasn't implemented for anything
that I'm aware of. Good luck.
